COMMUNITY SERVICES PROGRAMS FOR PEOPLE WITH PHYSICAL DISABILITIES (CSPPPD)

This is a program of the Bureau of Home and Community Based Services, Department of Public Welfare. Liberty Resources serves as the contractor for the two regions comprising the five counties of Southeast Pennsylvania and the 15 counties of Northeast Pennsylvania. The CSPPPD is Pennsylvania's response to nursing home reform mandated by OBRA‑87 Federal legislation.

The Community Services Program for Persons with Physical Disabilities (CSPPPD) encompasses three Medicaid Waiver Programs that provide a variety of services to persons with developmental disabilities (other than mental retardation), acquired disabilities and head injuries.

The OBRA Waiver

The OBRA waiver serves individuals with developmental disabilities other than mental retardation. An eligible individual:

• is a person with a disability that occurred before the age of 22
• has a disability that is expected to continue indefinitely
• has a disability that results in three or more limitations in the following areas:
• self care
• understanding and using language
• learning
• self direction
• capacity for Independent Living
• mobility
• lives in the nursing home and wants to transition to the community

OBRA services include supports coordination, daily living (attendant care services), respite, home modifications, specialized equipment and supplies, personal emergency response systems, skills training, peer counseling, prevocational/supported employment and educational services, therapies, adult day services, and transportation.

The Independence Waiver

The Independence Waiver serves individuals with acquired physical disabilities. An eligible individual:

• is 18 years of age or older
• has substantial limitations in three or more of the following areas:
• self care
• understanding and using language
• learning
• self direction
• capacity for Independent Living
• mobility
• requires two or more services to live in the community
• does not require mechanical ventilator support
• lives in the nursing home and wants to transition to the community

Independence Waiver services include service coordination, daily living (attendant care services), respite, home modifications, specialized equipment and supplies, personal emergency response systems, skills training, peer counseling, educational services, therapies, and transportation.

The CommCare Waiver

The CommCare Waiver serves individuals with traumatic brain injuries. Those with other types of head injuries can be served under OBRA or Independence. An eligible individual:

• is 21 years of age or older
• has function limitation in three or more of the following areas
• mobility
• behavior
• communication
• self care
• self-direction
• capacity for Independent Living
• judgment, memory, and reasoning
• has a medical diagnosis of traumatic brain injury
• is not dependent on mechanical ventilator support

Services in CommCare include supports coordination, attendant care services, respite, prevocational and supported employment services, habilitation and support, educational services, home modifications, non-medical transportation, specialized equipment, chore service, emergency response systems, therapies, coaching and cueing, night supervision, structured day programs, behavior and cognitive therapy, family and Consumer counseling, skills training, and peer counseling.
