The Computer Assisted Dispatch System is a voluntary program for persons with disabilities in Philadelphia. By completing this form, you can alert the Philadelphia Police and Fire Departments that you or someone in your household has a disability and may require additional assistance in the event of an emergency.

To participate in this program please complete, sign, and return the Computer Assisted Dispatch (CAD) Information Form to the Philadelphia Police Department. As a service to our consumers, the Computer Assisted Dispatch (CAD) Information Form is available to be printed from this Web site in PDF format. The form is also available in large print, audiocassette, Braille, and Spanish by calling Sgt. Joseph Spera, Police Radio Training at 215-685-3940 (voice) or 215-685-3943 (TDD/TTY).
